If you had a paper accepted for the 2020 Annual Meeting, and wish to present it

at the 2021 Annual Meeting, please fill out this form.  Any proposal which was accepted

 for the 2020 Annual Meeting is automatically accepted for the 2021 Annual Meeting.

If you do not wish to present your 2020 proposal, you do not need to fill out this form

in order to decline the invitation to present. We will only include a 2020 proposal

in the 2021 Annual Meeting upon request.
